To apply for the Kerala Mandahasam scheme, one must download the application form online and submit it to the concerned officer. Follow the steps discussed to download the application form and submit it.1) Visit the official website (http://sjd.kerala.gov.in/) and click on the scheme option on the homepage.
2) From the list of government schemes, click on Mandahasam – Scheme to provide artificial dentures to Senior Citizens option.
3) Select Application Forms – Mandahasam Scheme under the document section and download the application form.
4) Make sure to enter all the correct details in the application form and submit the application to the concerned officer of the village.
5) Verification will be carried out by higher authorities, and after that, the beneficiaries will get artificial dentures and other scheme benefits.
Eligibility Criteria for the Mandahasam Scheme
Eligibility for the Mandahasam Scheme is applicable, and the applicant must possess the following requirements,- Residents of state – People who are permanent state residents are eligible to avail of the scheme benefits.
- Senior citizens Only – Senior citizens are only eligible to register for the scheme
- Must Belong to BPL level – The person applying for the scheme should belong below the poverty level
- Must fall under Income limit – Income limit has been set for the scheme, and the concerned officer should authorize the income certificate
Documents Required For Kerala Mandahasam Scheme
The applicants must submit the prescribed documents while applying for the scheme, and the documents include,- Identification details – An identification card like an Aadhaar card, Voter ID card, recent photograph and other identification cards will be accepted
- Fitness certificate – The applicant must submit a fitness certificate relating to Oral Rehabilitation from a certified dentist
- Residential documents – The candidate must justify that they are natives of the states to be eligible and to get the scheme's benefits.
- BPL certificate – The applicant must fall under the BPL and should submit a suitable BPL certificate at the time of registration
- Income certificate – The candidate should submit an income certificate at the time of registering as the certificate, and the application will be scrutinized by higher authorities to ensure the elderly person is eligible
