Being one of the oldest in Telangana, the temple was constructed by Bhakta Ramdas's uncles Akkanna and Madanna. Tradition has it that a devotee who used to travel to Tirupati each year was prevented from doing so once due to a terrible illness. In his dream, Lord Venkateshwara showed him and stated, "I am right here in the neighbouring jungle. You need not be concerned. The devotee immediately moved to the location that the Lord had shown him in the dream, where he discovered a molehill that he had dug up. Accidentally striking Lord Balaji's idol beneath the chin and on the chest with the axe caused unexpectedly copious amounts of blood to pour from the "wounds," covering the area and colouring it scarlet. When he saw this, the devotee could not believe what he was seeing. When he suddenly heard a voice saying, "Flood the molehill with cow's milk," he could not believe his ears. A Swayambhu idol of Lord Balaji with Sridevi and Bhoodevi (a rare combination) was discovered when the devotee accomplished this, and the idol was put with the proper formalities, and a temple was built for it.

As the Pratyaksha Daiva in the Kaliyuga, Sri Balaji Venkateshwara is therefore accessible at Chilkur to bestow blessings on His devotees who, for any reason, are unable to travel to Tirupati. Many devoted worshipers visit the temple throughout the year, especially during the Poolangi, Annakota, and Brahmothsavams, to obtain the Lord's blessings and those of his consorts.

The idol of Ammavaru was placed in 1963, the year after the Chinese attack, with the sincere intention of restoring the temple's former splendour and importance. When the aggression was unilaterally vacated, Ammavaru was given the name Rajya Lakshmi to denote this happy occurrence. The distinctive characteristic of this idol is that it has four hands, the fourth of which is positioned such that it faces the lotus feet, representing the Saranagathi ideology, while the other three hands hold lotus flowers.

Great Acharyas have occasionally visited the shrine. Every time the Jeer of the Sri Ahobila Mutt travels to the twin cities, he must stop by the temple, where the first Jeer's idol is housed. Sri Vallabhacharya Sampradaya Tilakayaths have started frequenting the temple. The trustees' efforts to improve the temple were honoured by Jagadguru Sri Sankaracharya of the Sringeri Mutt and his disciple.
